Nail lichen planus — מעורבות ציפורניים ב-10% מחולי LP, עלולה להיות isolated. שינויים: thinning, ridging, pterygium (dorsal), onycholysis. ⚠️ עלול לגרום scarring + permanent nail loss. Bologna Ch 97.

Thinning of nail plate + longitudinal ridging/grooving (90%). Pterygium (dorsal) — adhesion of proximal nail fold to matrix/nail bed with V-shaped scarring (pathognomonic!). Onychorrhexis (brittle, splitting). Onycholysis. ⚠️ Twenty-nail dystrophy (trachyonychia) — rough, sandpaper nails, all 20. Red lunulae. Complete nail destruction in severe cases.

-ביופסיה (nail matrix biopsy — when diagnosis uncertain): interface dermatitis, band-like lymphocytic infiltrate at dermoepidermal junction, Max Joseph spaces, hypergranulosis, civatte bodies. ⚠️ Biopsy matrix — risk of permanent nail dystrophy, only when clinically uncertain.

-DDx: Psoriasis (pitting, oil spots, subungual hyperkeratosis — ⚠️ no pterygium), Onychomycosis (KOH/culture, no matrix scarring), Alopecia areata nails (fine pitting, no pterygium), Lichen striatus (single digit, children), Idiopathic trachyonychia (no scarring). ⚠️ Pterygium = LP until proven otherwise.

-טיפול: Early/aggressive to prevent irreversible scarring! IL triamcinolone injection (2.5-5 מ"ג/מ"ל) into proximal nail fold/matrix — every 4-6 weeks, 3-6 sessions (first-line for limited). Potent TCS under occlusion (clobetasol 0.05% to nail fold). Systemic (widespread/severe): Prednisone 0.5-1 מ"ג/ק"ג taper 3-6 months. Acitretin 20-30 מ"ג/יום. ⚠️ If pterygium established — irreversible, treatment prevents progression only.
