Actinic Prurigo — hereditary photodermatosis with strong HLA-DR4 (DRB1*0407) association. שכיחה ב-Latin American/Native American/Mestizo populations. מתחילה בילדות. UVA-triggered. מאפיין: cheilitis (lip involvement) פתוגנומוני. DDx: PLE, CAD, lupus. מהלך כרוני עם flares בקיץ. שיפור מסוים עם הגיל.

📝 אנמנזה

רגישות לשמש מילדות. פריחה עם גרד לאחר חשיפה לשמש. Cheilitis (שפתיים יבשות, סדוקות, crusted). היסטוריה משפחתית (בעיקר Latin American descent). עונתיות (קיץ). כתמים כהים/צלקות. מעורבות עיניים (conjunctivitis). ⚠️ אינה חולפת לאחר hardening (בניגוד ל-PLE).

🔍 בדיקה

Eczematous papules/plaques/nodules על אזורים חשופים (פנים, V-neck, זרועות). ⚠️ Cheilitis — crusting + erosions + edema of lower lip (פתוגנומוני, מבדיל מ-PLE). Conjunctivitis — pseudopterygium. Excoriations, lichenification (chronic scratching). Post-inflammatory hyper/hypopigmentation. Scarring.

🔬 בירור

-בירור: HLA-DRB1*0407 typing (positive ב-60-90%). Phototesting: reduced MED (minimal erythema dose) ל-UVA ± UVB. Provocation test: UVA exposure → papules ב-48-72 שעות (positive ב-60%). ANA, anti-Ro/La — שליליים (DDx lupus). ביופסיה: spongiosis + lymphocytic infiltrate + lymphoid follicles בדרמיס (characteristic!).

-הגנה מהשמש: SPF 50+ broad-spectrum (UVA+UVB) + בגדים מגנים + UV-blocking window film. ⚠️ קרם הגנה לבד לא מספיק (UVA penetrates glass).

-טיפול — קו ראשון: NB-UVB prophylactic desensitization (spring course, 15-20 sessions). TCS (Betamethasone valerate 0.1%) / TCI (Tacrolimus 0.1%) לנגעים. Lip: Tacrolimus 0.1% ointment to lips x2/d.

-קו שני (moderate-severe): Thalidomide 50-200 מג/d — ⚠️ MOST EFFECTIVE treatment but teratogenic! Mandatory pregnancy prevention. ת"ל: neuropathy, somnolence. Cyclosporine 3-5 מג/ק"ג/d (alternative to thalidomide).

-קו שלישי: Azathioprine 1-2.5 מג/ק"ג/d (steroid-sparing). Antimalarials (Hydroxychloroquine 200 מג x2/d — less effective than PLE). Omalizumab (case reports).
