📖 מבוא

Livedoid vasculopathy (atrophie blanche) — thrombotic vasculopathy (NOT vasculitis!) of dermal vessels. Pathogenesis: microvascular thrombosis → ischemic infarction → porcelain-white stellate scars (atrophie blanche). פעילות עונתית: קיץ > חורף. כואבת מאוד. שכיחה בנשים צעירות. Risk factors: thrombophilia, hyperhomocysteinemia, protein C/S deficiency, antiphospholipid syndrome, Factor V Leiden.

📝 אנמנזה

כיבים כואבים חוזרים ב-lower legs/ankles. Purpuric macules שהופכות לכיבים. צלקות לבנות atrophic (atrophie blanche). כאב — disproportionate to size! עונתיות (גרוע יותר בקיץ). Livedo racemosa מסביב. מחלות רקע: SLE, APS, thrombophilia. תרופות: OCP. היסטוריה של DVT/PE.

🔍 בדיקה

Lower legs/ankles: Livedo racemosa — reticular violaceous pattern. Purpuric macules/papules → painful small ulcers (punched-out, irregular borders). Atrophie blanche — porcelain-white stellate scars with surrounding hyperpigmentation and telangiectasia (pathognomonic). ⚠️ אין palpable purpura (מבדיל מ-vasculitis).

-קו שני: Pentoxifylline 400 מג x3/d (improves blood flow). Tissue Plasminogen Activator (tPA) — IV infusion in severe cases. Danazol 200 מג x2/d (increases fibrinolysis). Hyperbaric oxygen therapy (refractory).

-טיפול — Anticoagulation/Antithrombotic (first-line): Rivaroxaban 10-15 מג/יום — evidence growing (multiple case series). Low-dose Aspirin 100 מג/d + Dipyridamole 75 מג x3/d. Warfarin (INR 2-3). Low-molecular-weight Heparin (bridging in flares).

-טיפול מקומי: חבישות לחות, pain management. Compression stockings (30-40 mmHg). Wound care: hydrogel/foam dressings.
