Palmoplantar Keratoderma (PPK) — עיבוי (hyperkeratosis) של כפות הידיים ו/או כפות הרגליים. תורשתי (hereditary): Diffuse (Thost-Unna — AD, keratin 1/9; Vörner — epidermolytic), Focal (striate), Punctate. נרכש (acquired): Eczema, psoriasis, tinea, PRP, drugs (arsenic, lithium), paraneoplastic (Tripe palms — gastric CA), Howel-Evans syndrome (esophageal CA + PPK). אבחנה מבדלת רחבה — בירור שיטתי.

📝 אנמנזה

גיל הופעה (מלידה — hereditary, מאוחר — acquired). סיפור משפחתי (AD/AR). סימפטומים: כאב בהליכה, סדקים, זיהומים. מחלות עור: psoriasis, eczema, PRP. תרופות. סימנים סיסטמיים (ירידה במשקל — paraneoplastic). Hyperhidrosis נלווה.

🔍 בדיקה

Diffuse: hyperkeratosis אחידה של כל כף הרגל/יד, transgrediens (extending to dorsum) או non-transgrediens. Focal/striate: pressure points, linear streaks. Punctate: tiny keratotic papules. KOH — שלילת tinea (DDx מרכזי!).

-PPK נרכש — טיפול בגורם: Psoriasis — calcipotriol/betamethasone, biologics. Eczema — TCS, emollients. Tinea — terbinafine. Drug-induced — הפסקת תרופה.
