Port-Wine Stain (PWS, Capillary Malformation) — מלפורמציה קפילרית מולדת, נוכחת מלידה ואינה חולפת ספונטנית (להבדיל מ-nevus simplex/salmon patch). שכיחות 0.3%. מסווגת בסיווג ISSVA כ-capillary malformation. PWS ב-V1 distribution — סיכון ל-Sturge-Weber syndrome (leptomeningeal angiomatosis, glaucoma). מוטציה סומטית ב-GNAQ.

📝 אנמנזה

נוכח מלידה. גודל ומיקום (V1 — Sturge-Weber?). שינוי בגודל / עובי עם הגיל (progressive thickening). פרכוסים, עיכוב התפתחותי (SWS). בעיות עיניים / גלאוקומה. דימומים. סיפור משפחתי.

🔍 בדיקה

כתם שטוח ורוד עד סגול, unilateral, sharp midline border. גבולות ברורים. אינו נעלם בלחץ חלקי (capillary — slow refill). עם הגיל — darkening, thickening, cobblestone appearance, pyogenic granuloma-like nodules. Soft tissue / skeletal hypertrophy.

-PWS עמידים ללייזר (70-80% persistent): Nd:YAG 1064nm, Alexandrite 755nm. Rapamycin topical 0.1% (investigational) — anti-angiogenic. Photodynamic therapy (PDT) בחקר.

-Hypertrophy/nodularity: CO2 laser או shaving. Camouflage cosmetics — Dermablend, Covermark.
